Your First Office Visit

As a new patient, we would like you to complete a short but thorough health questionaire. This will help us to provide you with the best possible care. A single upper and lower jaw x-ray (panoramic view) may be taken. Appropriate additional radiographic and diagnostic studies will be performed as needed.

To help us speed up the process, we would like to ask you to fill out the following forms, print them and bring them with you. We are working to enable them for online submission very soon. All four are necessary, and will save you time at our office.

Dr. Wells maintains staff privileges at these hospitals:

  • St. Luke's Hospital of Jacksonville
  • Memorial Hospital of Jacksonville
  • Shands Hospital of Jacksonville
  • Flagler Hospital of St. Augustine

Outpatient care is also available at each of these facilities and at selected, free-standing surgicenters.
